At anytime when i have a nightmare-Mistress cuddles me in close so i know that i am safe. Leaning over him while he's lying down, for instance, doesn't work very well for me. If I sit on the end of the bed or (even better) kneel in front of him while he stands, my throat is more... open, and it seems to be at just the right angle for me to take him all the way. Keep trying different positions and see what works. ;) I’ve found that my gag reflex is stronger depending on the position I’m in.
